Shadow Health Assessment

The health assessment is an important component of advanced nurse practice because it helps in gathering detailed information about the patient that is then used in developing the treatment plan. The advanced nurse practitioner must be sensitive while interacting with the patients to create an environment where the patient is free to provide all the necessary information needed to develop the right diagnosis. The purpose of this assignment is conduct a comprehensive history and collecting data to assess Ms. Jones’ recent right foot injury. The assignment will highlight the effective therapeutic communication, identify the problems in the patient’s case with enough evidence from the data, differentiate care and develop the best way to addressing the patient’s health issue.

Health History Communication, Education, Empathy and Summary

The interview was conducted with Ms. Tina Jones who presented to the clinic complaining of the right foot injury. The interview focused on gathering data that would help in developing the right diagnosis about the patient’s condition. The first step in the interview was to create rapport with the patient to facilitate a free and interactive session. The questions covered in the in the interview were non-sensitive and so the patient found it easy to respond to them. The patient was also educated on the need to have regular and non-intense physical exercise. Also, the patient was told not to strain the injured ankle because this could worsen her condition. If necessary, the patient should use walking aids to minimize pain and strain on the already injured tissues.

 

Subjective Data

CC: “right foot injury”

HPI: T. J presented to the clinic with the complaint of right foot injury. The patient rated her pain 5/10 and did not have any problems with the joint. The patient denied having any muscle weaknesses or pain before. The patient indicated that he had been stuck sitting for the last few days because her foot hurts so much.

Medications: No past medications indicated.
PMH: The patient has no past medical history.
FH: The patient’s father and brother have obesity.
SH:  The patient denies smoking, or consuming alcohol. She is a committed member of her local church.

Allergies: NKDA, reacts to paints and chemicals.

Immunizations: n/a
ROS
General-The patient is alert and oriented. However, she feel unhappy about her paining leg. The patient corporates well and is able to answer all the questions during the interview.

HEENT: normal visual acuity (20/60 in both eyes). The tympanic membranes are intact. No head injury. No swelling in the lymph nodes.
Cardiovascular-Negative chest pain. No palpitations.
Gastrointestinal– No nausea, non-distended abdomen.
Pulmonary– negative for dyspnea.

Musculoskeletal: The patient feels pain while walking. Ankle strength, right 4/5,left 5/5.

 Objective Data

VS: BP125/85 H/R 68 R 18 T97.8

General-The patient is concerned with the pain in her right leg.

Cardiovascular– No chest pain or cyanosis.

Gastrointestinal-The abdomen non-tender, non-distended and symmetrical.

Pulmonary– Lungs are clear to auscultation with no chest pain or murmuring. The patient coughs and has bruising injuries on his chest.

Musculoskeletal: Limited ROM on the right leg.

Abnormal findings

  • Reports difficulties standing up
  • Feels a lot of pain on standing or walking (Ragab et al., 2017).
  • The patient has fever
  • The right ankle is inflamed

Differential Diagnosis

Arthritis: The condition is characterized by tenderness and swelling in the joints. The common signs and symptoms include reduced range of motion, pain and swelling in the knees. The patient could be having arthritis considering that she does not have a history of physical injury or intense exercise (Guo et al., 2018).

Gout: The condition is characterized by an intense joint pain and inflammation which is present in the patient’s ankle. Also, the patient has a limited range of motion which is consistent with gout signs and symptoms (Ragab et al., 2017).

Achilles tendon injury: The condition is characterized by tendon pathologies such as rapture and inflammation (Marques, 2019). Patients with the condition experience pain that worsen with activity. Also, they may have knee swelling. Though, the patient does not indicate having engaged in intense physical exercise that could have resulted in the tendon injury.

Plan

Further tests will be conducted on the patient. The tests include rheumatoid factor test, uric acid level test and an x-ray to determine whether there are tendon injuries. The nursing plan will be to manage the patient’s pain. The patient will be initiated on acetaminophen 500mg PO TDs.

Self-Reflection: Tina Jones musculoskeletal

 

The patient’s assessment provided an opportunity to explore the various factors likely to influence the illness. The patient came with the complaint of right foot injury; however, conducting a comprehensive assessment helps in determining whether she could be having other underlying conditions. There were a lot to learn from the experience ranging from the interaction with patients and ability to overcome cross-cultural barriers. My first goal was to identify the patient’s social and cultural values to minimize the possibilities of conflicts arising. The patient was cooperative and this is something that any person would be happy about; though, the healthcare providers are trained to interact and handle people from different backgrounds and experiences. Furthermore, collecting the family history is essential as it guides the healthcare provider on the possible diagnosis of the patient’s illness. The interview session was elaborative and at one point, time was a limiting factor. I plan to improve on managing time during such sessions in future by controlling the interaction with the patient.
